Background:
STP-172N(45)IG of City of Broken Arrows Project # ST0914 - 23rd Street Improvements from Houston Street to Kenosha Street project includes: pavement widening from 2-lanes to 5-lanes with curb and gutter, stormwater improvements, waterline improvements, sanitary sewer improvements, addition of both sidewalk and side path, new traffic signals at 23rd & Houston, and construction of a new bridge over SH51 (Broken Arrow Expressway). Due to federal funding being utilized, the project was bid and awarded by the Oklahoma Department of Transportation (ODOT) with Sherwood Construction being awarded the job. The project is currently under construction with the contractor being overseen by ODOT's construction management consultant Benchmark.
During construction, several issues have arisen which require a change order to the executed as bid contract.
Change Order #5 addresses the following additions/deletions/changes to the contract:
- Provided and installed additional epoxy rebar in order to complete approach slabs on Bridge A. This was due to an incorrect calculation involving specification of #9 rebar on plan sheet(s) instead of #4 as described in rebar summary.
- Removal of center median street lighting on SH-51. Light was in conflict with Bridge A construction and City of Broken Arrow requested light be removed entirely. This resulted in lighting circuited requiring reestablishment. A pull box, 70 LF of 2" PVC conduit installed via boring method, and new electrical conductors were provided and installed as part of this scope as well as.
