Approval of and authorization to execute Change Order CO-5 for Construction Contract ST1507; 37th Street: Omaha to Albany and Rural Water District #4 Waterline

Background:
37th Street: Omaha to Albany and Wagoner County Rural Water District No. 4 (RWD4) 209th Waterline is a 2014 General Obligation Bond project. 37th Street between Omaha and Albany is new roadway construction. The project was advertised with N. 37th Street new road as the base bid and RWD4 209th Waterline relocation as Additive Alternate No. 1 (16" Waterline Upgrade) and Additive Alternate No. 2 (10" Waterline Like-in-Kind). The contract was awarded for the base bid and Additive Alternate No.1 in the amount of $3,415,623.00 ($3,212,321.00 after RWD4 reimbursement) to Paragon Contractors, L.L.C. on May 5th, 2017. Funding for the improvements are from the 2014 Streets GO Bond.
This Change Order #5 addresses the following additions to the contract:
During the construction of the storm sewer and roadway grading of the intersection of Omaha St. and 37th street, it was observed that the existing City of Broken Arrow waterline is too high in elevation and conflicts with the new storm box culvert on the east as well as the new bar ditch to the west of 37th street. Through this change order, the contractor is to lower approximately 220 linear feet of waterline at station 64+00 on 37th street. The water line is to be lowered to an elevation of 3 feet below both the new box culvert as well as the bar ditch along the alignment.
The associated line items for within the base bid will be extended through this change order and are noted within. As noted in prior change orders, due to increased product costs from time of bid, the 12" PVC Restrained Joints shall be changed ordered in an additional $19.15 per linear foot.
