End
Background:
The Rose District Mill and Overlay project is a 2008 GO Bond street project. The work includes street repair along Commercial and Dallas Streets from Ash Avenue to First Street, and El Paso and Fort Worth Streets from Main Street to First Street. The project will consist of street maintenance for asphalt, Portland Cement and combined streets in Downtown Broken Arrow, to include patching, milling, and asphalt overlay. The project was advertised for bids on August 1 and 8, 2017 with the low bidder was Ellsworth Construction, LLC, with a bid of $248,055.80.

Funding for the improvements will be from the 2008 GO Bond.

This Change Order #1 addresses the following additions to the contract:

1. Total Number of Days Added by this Change Order:
- None at this time

2. Due to unforeseen issues with the subgrade under the entire area of the concrete pavement replacement on West Dallas St., the contract is instructed to install appropriate geo-grid as approved by the project engineer. The additional aggregate and placement shall be paid through the contract values established. The costs below include all materials, labor, and applicable markups to complete the scope above.

3. As directed by the Work Change Directive #1, the contractor is to remobilize and complete the additional scope of work for Commercial and Birch streets. The costs below summarize the additional costs for remobilization and traffic control to complete this work scoped in WCD #1.
