Background:
Leisure Park I Mill and Overlay is a 2011 General Obligation Bond project. Leisure Park subdivision, located west of Elm Place and north of Florence Street, has approximately 6,700 linear feet of asphalt streets. The project improvements were designed in-house by the Engineering and Construction Department. The project was advertised for bids on September 7th and 14th, 2016, and bids were opened on September 29, 2016. The low bid of $718,622.24 was submitted by H&G Paving Contractors. Funding for the improvements is from the 2011 GO Bond.

This Change Order #3 addresses the following additions to the contract:

- Total Number of Days Added by this Change Order:
- Due to additional coordination and installation of the Mailbox, the contract time shall be amended to include 2 calendar days of additional time.

- Contract Changes

1) Due to conflicts during construction resulting in the demolition of a property mailbox at the address 812 Waco St. The mailbox shall be purchased and installed similar to the mailbox that was removed. The cost below summarizes all materials, labor, and markups required to install the product complete in place.

The additional cost associated with these changes is as follows:
