title
Approval of and authorization to execute an Addendum to the Zero Card Service Fee in with the Zero Card, effective October 1, 2025
End

Background:
Zero Card is an optional benefit which was added to our existing health insurance plan in 2022. Zero provides cost savings to the Plan and to the employees who utilize the service. Zero offers no deductibles, no copays and no coinsurance when an employee chooses to utilize a covered provider for labs, imaging, physical therapy, general surgery, gastrointestinal, orthopedic, spine, ENT, pain management, sleep studies, women's health and more. The City of Broken Arrow eligible participants have been utilizing Zero services since September of 2022.
Zero works with self-funded employer health plans to help employers lower the cost of scheduled care, and gives members access to care for $0. Team Zero provides a team of Personal Health Assistants which offer concierge support to help match plan members with the highest-quality, most affordable providers.
There are no setup fees, no ongoing fees and no per employee per month charges for this service. If an employee chooses to utilize the program at a participating provider, Zero bills the City for the discounted service and charges an 18% fee for the services, an increase of 3% from the previous contract.
Staff and the Insurance Advisory Committee unanimously recommend continued services with Zero to be utilized in conjunction with the employee health plan for eligible employees and dependents.
