Background:

Design for the improvements to the Broken Arrow maintenance facility have been completed and Engineering and Construction Department staff worked with the design consultant to prepare construction documents for letting. The project consists of approximately 11,600 SY full depth asphalt pavement replacement, sidewalk and ADA improvements, site grading, miscellaneous appurtenances as well as permanent and temporary stormwater treatment improvements. For this Phase 1 project the existing operations shall remain open and accessible throughout construction activities.

The project was advertised for bids on January 30th and February 6th, 2023, and ultimately the lowest responsible combined bid of $1,051,828.01 was submitted by Ellsworth Construction, LLC with a base bid of $991,818.51 and an additive alternate 1 bid of $60,009.50. The project has a Notice to Proceed scheduled for June 19th, 2023.

This Change Order #2 pertains to the following Scope/Time Changes:

During construction of the as directed new limits of the project as outlined within the Change Order #1 documents, it was found that several areas of subgrade were unsuitable for stability needs of the parking lot. As such additional excavation, stabilization, and fabric was needed to stabilize these areas of work. The attached amended quantities for the project encompass these changes as needed to finalize the installation of the project. The contract shall be amended to capture these revised quantities as attached.

Changes in Contract Time:

Substantial completion of the project shall be noted as November 17th and contract time shall be amended accordingly.

The additional cost associated with these changes is as follows:
