Background:

The 2011 Streets General Obligation Bond included a project described as “Village Square:  Repair, mill and overlay streets with curb repairs and construct handicap access ramps as required.”  Bonds for the project were sold in December 2014.  Engineering and Construction Staff prepared bidding documents, and the project was advertised for bids on October 5 and 12, 2015.  Bids were opened on October 27, 2015, and three (3) bids were received.  The low bidder was Pavement Conservation Specialists, Inc., with a proposal of $796,582.00.  Construction Notice to Proceed was issued January 11, 2016, and Substantial Completion was issued May 31, 2016.

 

This Change Order #1 addresses the following additions to the contract:

 

1.  Total Number of Days Added by this Change Order:                                                                                                         

    - Due to weather delays in the project, an extension of 21 days is requested                                                                                                         

2. Reconciling Changes: Due to site conditions varying from design parameters, field adjustments of the plan quantities by the City contract administrator was authorized. These changes in quantities are summarized on the attached "Project Final Quantities Report". The listed change order cost is to adjust the original contract value to the reflected installed contract value.
