Background

This project includes milling and resurfacing Iola Street from Elm Place to 1st Street with asphalt as well as full-depth patches as necessary.  The project also includes construction of approximately 712 square yards of sidewalk, ADA curb ramps at intersections, and concrete work at driveway returns. This roadway reconstruction will be located on Iola Street from Elm Place to North 1st Street in Broken Arrow, OK.

The project was advertised on March 12 and March 19, 2024. Bids were opened on March 26, 2024 with six (6) bids received with the lowest responsible bidder being Tri-Star Construction, LLC, with a Total Bid of $293,000.00.

This Change Order No. 1 addresses the following additions to the contract:

                     At the direction of COBA engineering and construction staff, additional quantities of repair have been identified for work above and beyond the original quantity within the contract, but still within the scope of the project work within the Iola Street Rehabilitation from Elm Place to North 1st Street project. These items include additional full-depth asphalt pavement patches due to unforeseen grade and pavement conditions, subgrade modifications, and additional driveway repair, curb and gutter, sidewalk and ADA ramps. The line item quantities noted within this change order shall be extended such to contractually cover the associated additional work. A final reconcile change order may be processed upon completion of the project to finalize the actual installed quantities should they vary from the attached. The attached forecasted quantities report illustrates these quantities changes.

Contract Time Adjustments:

                     Additional time consideration is not needed.
